Humana is seeking an Onsite Care Coordination Nurse in Augusta, GA, to assess and coordinate care for newly admitted members at assigned facilities.

The role combines in-person assessments with remote documentation in a hybrid work model, requiring GA residency within commuting distance. Eligible candidates will have RN licensure, discharge planning experience, and knowledge of EMR systems.

Qualificações

• Active Georgia RN license.
• 3+ years acute care experience and discharge planning exposure.

Responsabilidades

• Conduct face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning and coordinate follow-up with PCPs.
• Identify barriers to discharge and implement care coordination strategies.
• Collaborate with providers and care teams for timely, coordinated care.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately and maintain patient privacy.
