Humana Inc. is seeking an Onsite Care Coordination Nurse to support members during critical care moments, conducting face-to-face assessments and leading discharge coordination.

You will identify care gaps, partner with providers, and educate families to ensure safe transitions from hospital to home. The role blends in-person facility work with remote coordination, offering a meaningful hybrid schedule.

Qualifications

• Current, unrestricted Registered Nurse license in Texas.
• Must be located within a 30–60 minute drive to Shannon Medical Center.
• Minimum 3 years acute care nursing experience (medical-surgical, CCU, etc.).
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience with Microsoft Office and electronic medical records.
• Valid driver’s license and access to a personal vehicle.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members at acute care facilities.
• Support discharge planning from admission through post-discharge with follow-up visits.
• Identify gaps in care and barriers to discharge; implement improvement strategies.
• Collaborate with providers, facility case managers, and care teams for coordinated care.
• Educate members on diagnoses, medications, and preventive measures to reduce readmission.
• Complete all required documentation accurately and promptly.
• Maintain adherence to infection control, safety, and patient privacy standards.
