Job summary

Qualifications

• MA RN license in good standing required.
• Current BLS certification.
• Bio/chemo certificate required.
• ONS chemotherapy provider card within 60 days of hire; must obtain within 2 years.

Responsibilities

• Triages calls and coordinates care based on clinical acuity and guidelines.
• Performs vital signs, medication reconciliation, and documentation in EMR.
• Schedules appointments for PCPs and specialists.
• Reviews lab/x-ray results and communicates critical values to providers.
• Supports providers with education, documentation, and care planning.
• Maintains patient-centered care and collaborates with the team.
