Carease Health - WI is seeking a Nurse Practitioner or Physician Assistant to deliver preventative care to residents in skilled nursing facilities. This hybrid role includes on-site visits and remote work, supported by a scribe team for efficient documentation.

You will collaborate with a clinical team to develop care plans, perform assessments, and provide medical treatment and education, with strong emphasis on teamwork and patient communication.

Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.

Responsabilités

• Provide high-quality preventative care to residents via on-site visits and remote monitoring.
• Conduct patient assessments, order diagnostic tests, and develop personalized care plans.
• Collaborate with healthcare teams to improve patient outcomes and reduce hospitalizations.
• Communicate with patients, families, and providers to ensure seamless care transitions.
• Document patient encounters and treatment plans in a timely and accurate manner.
