Job summary

Carease Health is seeking an experienced Nurse Practitioner (NP) with a geriatrics/internal medicine focus to deliver preventative care to residents in skilled nursing facilities. You will perform on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation.

You will build strong relationships with facilities, maintain patient-centered care, and collaborate with staff, providers, and families to ensure comprehensive treatment.

Qualifications

• Active NP license in the state of practice.
• 1+ year of geriatrics care and documentation review.
• Excellent communication, collaboration, and teamwork skills.

Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities.
• Conduct on-site visits and remote work from home with a dedicated scribe team.
