You will provide on-site visits and work remotely with a dedicated scribe team to support documentation and care coordination.

Qualifications

• Active NP or PA license in the practicing state.
• 1+ year experience in geriatrics or internal medicine.
• Excellent communication and teamwork skills.
• Experience with chronic care management and remote monitoring is a plus.
