• Anchor the hybrid care team through Foley Catheter Insertion and Epic Systems cases from intake to follow-up.
• Round on assigned patients each shift and document findings in the electronic record.
• Perform diagnostic and screening procedures within scope of practice.
• Manage a hybrid panel of chronic patients and monitor empathy metrics.
