Mains’l is seeking a Licensed Practical Nurse (LPN) to join our Health and Wellness Team in a hybrid role. You will provide health support, wellness education, and staff training, while building relationships through in-home visits and phone consultations under an RN's direction.

You’ll collaborate with the RN and management, support new enrollments with medically complex individuals, and help coordinate care through provider referrals, ensuring quality care and staff confidence.

Qualifications

• Active LPN license in good standing with the Minnesota Board.
• Experience in home or community-based care preferred.
• Ability to clearly explain health information to staff and team.
• Strong time management and adaptability to changing needs.
• Collaborative, compassionate team player focused on wellbeing.
• Willingness to learn and pursue professional growth.

Responsabilités

• Conduct in-home visits to assess health and monitor wellness.
• Train direct support staff on medical tasks and wellness practices.
• Coordinate with RN and management on changing health needs.
• Support new enrollments with individuals with significant medical needs.
• Review provider referrals to assist care coordination.
• Participate in team meetings to enhance communication and quality of care.
