Job summary

Mains’l is seeking a Licensed Practical Nurse to join our Health and Wellness Team in a hybrid role, delivering in-home health support, wellness education, and staff training under an RN. You will build relationships through home visits, phone consultations, and team collaboration while supporting health needs of the people we serve.

You will monitor wellness, train staff on medical tasks, and coordinate with the RN and management on changes in health needs, including new enrollments and care

Qualifications

• Active LPN license in good standing with the Minnesota Board of Nursing.
• Experience in home or community-based care preferred.
• Strong communication skills to explain health information and procedures to staff.
• Organized and adaptable to manage multiple priorities.

Responsibilities

In this hybrid role, you’ll provide health support, wellness education, and staff training under the direction of a Registered Nurse (RN). You’ll build relationships through in-home visits, phone consultations, and collaboration with team members while helping staff feel confident supporting the health needs of the people we serve.

What You’ll Do

What You’ll Bring

• An active nursing license: Current Licensed Practical Nurse (LPN) license in good standing with the Minnesota Board of Nursing.

Compensation & Schedule

• Pay: $25.36–$28.80 per hour
• Schedule: Full-time, primarily daytime hours
• Work setting: Hybrid, with in-home visits, staff training, phone consultations, and team collaboration
