Join Mains’l in Monticello, MN as a Licensed Practical Nurse (LPN) on our Health and Wellness Team. In this hybrid, in-home role you’ll provide health support, wellness education, and staff training under the direction of an RN, building trusted relationships with individuals and families.

Qualifications

• Active LPN license in good standing with Minnesota Board.
• Experience in home or community-based care preferred.
• Strong communication skills to explain health information clearly.
• Ability to manage time and priorities in a flexible schedule.

Responsibilities

• Conduct in-home visits to gather health information and monitor wellness.
• Train direct support staff on medical procedures and wellness practices.
• Collaborate with the RN and management when health needs change.
• Support new enrollments involving individuals with significant medical needs.
• Review provider referrals to coordinate care and inform decisions.
• Participate in team meetings to strengthen communication and quality of care.
