Fresh Perspective Home Care is seeking a compassionate Licensed Practical Nurse (LPN) for a hybrid role supporting field and administrative operations in Portage, MI. You will conduct client assessments, develop and update care plans, provide caregiver training, and oversee care quality across teams.

Qualifications

• Active LPN license in good standing.
• Home Care, care coordination, or case management experience preferred.
• Strong communication, organizational, and problem-solving skills.

Responsibilities

• Provide nursing oversight and support for clients receiving skilled care services.
• Conduct initial and ongoing client assessments, including required reassessments.
• Develop, review, and update individualized care plans.
• Provide direct nursing services to clients as needed.
• Perform caregiver performance evaluations and quality assurance visits.
• Train, coach, and support caregivers through hands-on and ongoing education.
• Review caregiver documentation for accuracy and identify areas of concern.
• Monitor client satisfaction through check-in calls, service reviews, and surveys.
• Address client and caregiver concerns promptly and professionally.
• Maintain accurate documentation and ensure effective communication between clients, caregivers, and office staff.
• Collaborate with the Director of Nursing and interdisciplinary team to ensure quality care and compliance with agency standards.
