Role Description

This is a full-time, Hybrid Licensed Practical Nurse (LPN) role based in Portage (and surrounding areas), MI. The LPN provides direct one on one nursing care in clients’ homes, assessments, caregiver training, and maintaining care plans. The role involves a combination of field and office work. The LPN collaborates with caregivers, families, and other health professionals to promote safety, comfort, and independence for each client. Additional responsibilities include educating clients and families on health-related topics, maintaining infection control standards, and supporting a compassionate, respectful care environment.

Qualifications

• Active, unrestricted Licensed Practical Nurse (LPN) license in the state of Michigan and completion of an accredited practical nursing program.
• Demonstrated skills in medication administration, basic wound care, vital sign monitoring, and documentation of nursing care.
• Ability to provide person-centered care, including support with activities of daily living, chronic condition management, and dementia care.
• Strong communication and interpersonal skills to work effectively with clients, families, caregivers, and interdisciplinary healthcare teams.
• Commitment to maintaining client safety, infection control standards, and compliance with applicable regulations and agency policies.
• Reliable transportation for in-home visits in the Portage and Southwest Michigan area and ability to work a consistent full-time schedule.
• Experience is in home care, long-term care, or community-based nursing is preferred; CPR/First Aid certification and current immunizations are beneficial.
• Ability to work with diverse populations in a non-judgmental, culturally sensitive, and gender-neutral manner, respecting each individual’s dignity and choices.
