Job summary

CLS, Inc. in Wayne County, MI is seeking an Intake Specialist Mental Health Professional to coordinate and assess requests for services and supports, and to serve as Bridge Team Coordinator for Individual Plans of Service and transition services for newly enrolled individuals.

You will review access screenings, perform clinical assessments, coordinate with Supports Coordinators and service providers, and help families navigate resources while maintaining HIPAA privacy and confidentiality.

Qualifications

• Masters degree with LMSW/LLMSW or equivalent in Michigan.
• Minimum 3 years of community mental health experience.
• Experience developing Individual Plans of Service using person- and family-centered principles.

Responsibilities

• Review access screening and conduct a thorough assessment of preferences and needs.
• Coordinate intake processes, initial assessments, and required authorizations; HIPAA notifications.
• Meet individuals and families to determine living arrangements and supports; assess community resources.
• Perform clinical assessments for placement into community home settings.
• Coordinate planning with Supports Coordinators and advocates to ensure identified supports are addressed.
• Refer and enroll individuals with appropriate agencies and programs when needed.
• Support enrollment with DWIHN Consumer Enrollment program and initiate intake for residents of Wayne County.
