Qualifications

• Must be an LPN or RN with one year of paid professional experience or have a Baccalaureate degree with one year of paid professional experience with the aging or disabled population.
• Must be certified Case Manager or complete orientation with a certified Case Manager before attending certification training
• Have valid driver’s license and dependable transportation.

Responsibilities

• Identify member needs, goals and abilities
• Ensure ongoing coordination.
• Prepare and submit a comprehensive, written service plan for each member
• Implement certified ADvantage service plan
• Monitor delivery of services
• Identify high risk situations
• Orient and educate members
• Modify services to meet needs of members.
• Ensure confidentiality according to all relevant laws and regulations.
• Maintain adequate records
• Justifies expenditures and monitors and contains cost of plan.
• Assist in implantation of in-service education for field employees
