Job summary

Clover Health is seeking a compassionate Nurse Practitioner to join our Home Care Team in New Jersey, serving Medicare Advantage members in Essex County. This hybrid role blends in-home visits with telehealth to coordinate care for medically complex seniors.

You will perform assessments, manage chronic conditions, and develop personalized care plans while addressing social determinants of health. Strong communication and teamwork are essential to improve outcomes and reduce hospitalizations.

Qualifications

• Active NP license in New Jersey.
• National board certification as a Nurse Practitioner.
• Active DEA and NJ CDS registration.
• Experience with adult/geriatric populations in primary or home-based care.
• Strong clinical assessment and chronic disease management.
• Comfort with telehealth platforms.
• Excellent communication and collaboration skills.

Responsibilities

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Perform comprehensive assessments, medication reviews, and chronic disease management.
• Develop and implement individualized care plans addressing clinical and social determinants of health.
• Identify barriers to care including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence.
• Collaborate with physicians, care coordinators, social workers, and other team members.
• Leverage Clover Assistant and EMR to support proactive care management and close gaps in care.
• Monitor health status and intervene early to prevent ED visits and hospitalizations.
• Educate members and caregivers on disease management and preventive care.
