Job summary

Clover Health in Georgia is seeking a compassionate and driven Nurse Practitioner to join our Clover Home Care Team. This hybrid role delivers Medicare Advantage care through in-home visits and telehealth across Savannah, Athens, Rome, Macon, Augusta, and Atlanta markets, building strong patient relationships and improving outcomes for medically complex senior members.

Qualifications

• Active Nurse Practitioner license in Georgia or ability to obtain licensure.
• DEA registration required.
• National board certification as a Nurse Practitioner.
• Experience caring for adult or geriatric populations in primary/complex/home-based care settings.
• Strong clinical assessment and chronic disease management skills.
• Comfort with technology and telehealth platforms.

Responsibilities

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Perform comprehensive assessments, medication reviews, and chronic disease management.
• Develop and implement individualized care plans addressing clinical and social determinants of health.
• Identify barriers to care including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence.
• Collaborate with physicians, care coordinators, social workers, and other team members to deliver holistic, member-centered care.
• Utilize Clover Assistant and EMR to support proactive care management and close care gaps.
• Monitor health status and intervene to prevent avoidable ER visits and hospitalizations.
• Educate members and caregivers on disease management and preventive care.
