Job summary

Greater Lawrence Family Health Center, Inc. seeks a Home Visiting Program Coordinator to manage day-to-day operations of the program, acting as the liaison for patients and families, and supporting clinicians during home visits.

The role blends remote work with site attendance as needed, focusing on care coordination and documentation. The Coordinator will triage patient needs, coordinate scheduling, and assist with care transitions while ensuring smooth communication across the integrated care

Qualifications

• High School diploma or GED required; associate degree preferred.
• 2–4 years of administrative/clinical support in a clinical or healthcare setting.
• Experienced in care coordination and transition of care management.
• Able to work independently and manage time-sensitive tasks.
• Strong organizational skills and attention to detail.
• Proficient with EMR systems and Microsoft Office.

Responsibilities

• Serve as primary contact for patients, families, and caregivers.
• Coordinate in-home visits with clinicians and schedule visits.
• Create/populate patient medical records; verify insurance details.
• Process consent forms and patient documents prior to visits.
• Schedule visits per clinician needs and patient preferences; make reminder calls.
• Support care transitions and monitor follow-ups after hospital events.
• Coordinate with DME vendors and medical necessity/pre-authorization paperwork.
• Route faxes and document actions in the medical record.
