Greater Lawrence Family Health Center, Inc. is seeking an RN Home Health Program Nurse Coordinator to oversee the referral process for home health services and act as the primary liaison among medical providers, patients, and home health agencies.

This role focuses on ensuring timely initiation and appropriate utilization while supporting value-based care and continuity of care across sites. The position requires a Massachusetts RN license, a Bachelor’s degree in Nursing, and a minimum of seven

Qualifications

• Minimum of seven years of clinical nursing experience.
• At least three years of home health experience.
• Experience in utilization management is highly desirable.
• Proven ability to build strong professional relationships with Home Health providers.

Responsibilities

• Serve as central access point for home health referrals and assess medical necessity.
• Coordinate with providers and care teams to obtain required documentation.
• Monitor referrals, track patient progress, and ensure timely start of services.
• Escalate safety concerns or delays to leadership and ensure adherence to the care plan.
• Collaborate with care teams to coordinate transitions and educate patients and caregivers.
