Job summary

BAYADA Home Health Care, Inc. in Englishtown, New Jersey seeks a Home Health Physical Therapist for a full-time, hybrid role serving the Central NJ Senior Living team.

The position involves home visits to adult and geriatric clients in communities in Monmouth County, with potential office-based days and fieldwork in the home health setting. The PT will perform assessments, develop and monitor care plans, educate clients and families, and collaborate with a multidisciplinary team while using EMR

Qualifications

• NJ Physical Therapist-PT license required.
• Minimum 1 year experience as Physical Therapist-PT.
• Graduation from a program approved by APTA/CAAHEA as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, gait and balance.
• Implement wound care interventions using evidence-based dressings.
• Develop and adjust individualized care plans including manual therapy and gait training.
• Collaborate with nursing on multidisciplinary care plans as appropriate.
• Educate clients and families on rehabilitative care for independence.
• Document observations and interventions in EMR.
