Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a full-time, hybrid role serving Bound Brook, NJ. You will perform home visits, conduct mobility and gait assessments, and implement individualized PT plans for adult and geriatric clients in Somerset/Middlesex counties.

The position emphasizes independent work, strong communication, and use of EMR systems; a NJ PT license and ~1 year of PT experience are required. Sign-on bonus offered.

Qualifications

• Active New Jersey Physical Therapist license (PT).
• Minimum of 1 year recent PT experience.
• Graduation from an APTA/CAAHEA-accredited program or equivalent.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Conduct mobility, strength, balance, and gait assessments.
• Implement wound care and protective interventions.
• Develop and update individualized care plans with PT interventions.
• Educate clients and families on rehabilitative care for safety and independence.
• Document observations and care using EMR tablet.
