ParaMed Home Health Care is seeking an Occupational Therapist to deliver in-home therapy with a flexible, hybrid care model, combining in-person and virtual visits. Candidates should be licensed, with access to a vehicle, and able to engage families and caregivers in goal-focused care plans.

The role supports mentorship, ongoing development, and a collaborative team across Ontario. New grads welcome to apply, with paid vacation and comprehensive benefits in a unionized setting.

Formación

• Current active registration and in good standing as an Occupational Therapist with COTO.
• Must have a Valid Driver’s License and access to a reliable vehicle.
• Able to provide a current Vulnerable Sector Screening.

Responsabilidades

• Assess clients in their homes and develop an accurate therapy care plan based on the real environment.
• Develop health promotion programs and promote client safety.
• Prescribe equipment and supplies as part of therapy plan and collaborate with families and caregivers.
• Provide both face-to-face and virtual care, building a flexible schedule with clients.
• Collaborate with multi-disciplinary team and mentors to ensure high-quality care.
