Job summary

Mosaic is seeking a Mosaic at Home Coordinator to empower individuals with developmental disabilities in Topeka & Manhattan, KS. This role focuses on building meaningful connections, facilitating effective communication, and ensuring high-quality, person-centered support.

You will collaborate with families, home providers, and the planning team to develop personalized plans, monitor matches, and ensure contractual compliance while providing on-site and community-based support.

Qualifications

• Bachelor's Degree preferred.
• Minimum of two years of related work experience.
• Previous experience working with people with intellectual disabilities preferred.

Responsibilities

• Build meaningful connections with individuals, families/guardians, home providers and stakeholders, delivering exceptional service.
• Help individuals communicate goals, needs, and preferences.
• Monitor matches between individuals and home providers to ensure positive, supportive relationships.
• Collaborate with the service planning team to develop Individual Support Plans reflecting each person’s wishes and needs.
• Oversee contracted services and ensure compliance; conduct visits in home or service environments.
• Provide on-call support to Mosaic at Home providers and resolve issues.

Education
