Job summary

Mosaic at Home Coordinator position in Topeka and Manhattan KS is a hybrid role focused on empowering individuals with intellectual and developmental disabilities to live meaningful lives. You will build strong connections with individuals, families, guardians, home providers, and other stakeholders to ensure personalized support and a voice in daily life.

You will help people articulate goals, monitor relationships, and develop Individual Support Plans while ensuring policy and regulatory

Qualifications

• Bachelor's degree preferred.
• Minimum of two years of related work experience.
• Previous experience working with people with intellectual disabilities preferred.

Responsibilities

• Build meaningful connections with individuals, families/guardians, home providers and stakeholders to deliver exceptional service.
• Empower individuals to express goals and needs, ensuring their voices are heard and respected.
• Monitor relationships between individuals and home providers to ensure a positive match.
• Collaborate with the service planning team to create Individual Support Plans reflecting wishes while following policy.
• Oversee contracted services and complete visits in the home or service environment.
• Provide on-call support to Mosaic at Home Providers and resolve issues promptly.

Education
