Job summary

Mosaic is seeking a Mosaic at Home Coordinator in Topeka & Manhattan KS who will empower individuals with IDD by building meaningful connections with participants, families, and providers. You will help people express goals and ensure their voices shape their plans.

You will monitor matches, develop Individual Support Plans, oversee contracted services, and provide on-call support in home and community settings. A degree is preferred and experience with IDD is valued.

Qualifications

• Bachelor's degree preferred.
• Minimum two years of related work experience.
• Previous experience working with people with intellectual disabilities preferred.

Responsibilities

• Build meaningful connections with individuals receiving support, families/guardians, home providers and stakeholders.
• Empower individuals to express goals, dreams and needs and ensure their voices are heard.
• Monitor relationships between individuals and home providers to ensure positive matches.
• Develop Individual Support Plans with the service planning team reflecting each person's wishes and needs.
• Oversee contracted services, monitor delivery and complete visits in home or service environments.
• Provide on-call support to Mosaic at Home Providers and ensure consistent problem resolution.
