Job summary

Hawaii Medical Service Association seeks a Community Health Worker to advance health and wellness within Hawaiian communities by fostering trust, advocating for individuals and families, and connecting members with community services to address social determinants of health.

The role conducts outreach, coordinates care with licensed clinicians, supports closing care gaps, and collaborates with care managers to ensure timely access to programs and resources. Hybrid work in Hawaii is provided.

Qualifications

• Associate's degree and three years relevant healthcare/social services experience; or equivalent education/work experience.
• Fieldwork experience such as home visits, conducting health screenings in the community, and/or navigating social services.
• Excellent communication skills, both written and verbal.
• Basic working knowledge of Microsoft Office applications, including Word, Excel, and Outlook.
• Must have reliable transportation for in-person meetings with members.

Responsibilities

• Manage community health by establishing and maintaining trusting relationships with individuals, families, and providers to promote health and wellness.
• Coordinate and collaborate with licensed clinicians and Health Management programs to provide optimal care.
• Use motivational interviewing to encourage provider appointments and close care gaps.
• Deliver education, resources, referrals, and connections to health-related services.
• Guide community members navigating healthcare systems and programs, with respect to licensure boundaries.
• Provide culturally appropriate health education and instructions for using health services.
• Perform other duties as assigned.
