Placements24 is seeking an Elderly Care Specialist to join our team in Welkom, Free State. The role focuses on assessing needs, creating care plans, and coordinating services to support seniors in the community.

The position offers a hybrid arrangement balancing community interaction with remote administrative tasks. The ideal candidate has a minimum of 3 years in elderly care, strong geriatric knowledge, excellent communication, and a genuine passion for enhancing the lives of older adults.

Qualifications

Qualification in Nursing, Social Work, Gerontology, or related field.
Minimum 3 years' experience in elderly care.
Knowledge of age-related conditions and geriatric best practices.
Driver's license and reliable transport for client visits.

Responsibilities

Conduct comprehensive assessments of clients' physical, emotional, and social needs.
Develop, implement, and review personalized care plans with clients and families.
Coordinate services: personal care, medication support, meals, and transport.
Organize social and recreational activities to reduce isolation.
Monitor health and well-being; address changes promptly.
Be primary contact for clients and families with clear communication.
