Position Summary

Reporting to the Division Director of Day and Employment, the delegating RN provides nursing support in the form of case management for assigned individuals in The Arc’s Employment Services, Day, and Jaycee’s Daycare, participates actively as a member of individuals’ IP teams as needed, and teaches and supervises staff in the performance of delegated nursing tasks; This position has no direct reports.

Position Responsibilities

• Complete Initial Nursing Assessments within established timeframes, with updates as needed, for all assigned Meaningful Day participants. Assessments include the Self-Medication Assessment and the Choking/Aspiration Pneumonia Risk Assessment.
• Develop Nursing Care Plans and update them as needed for all individuals requiring delegated nursing functions.
• Complete nursing reviews of health status and delegated care (i.e., “45-day reviews”) for all individuals requiring delegated nursing functions. Reviews/assessments may be required more often according to individuals’ needs.
• Provide teaching and/or written verification of teaching and competency of staff in the performance of delegated nursing functions.
• Regularly review, at least annually, and document staff competency in the performance of delegated nursing functions.
• Complete all required nursing intake documentation for individuals entering Employment and Day Services, including review of medical and health records, medication information, nursing assessments, risk assessments, physician orders, and other applicable nursing documentation.
• Ensure all required nursing intake documentation is complete, accurate, and in place prior to the individual beginning services, in accordance with agency procedures and applicable DDA requirements.
• Provide call coverage by accepting calls from staff and provide directions as needed.
• Respond appropriately and promptly to emergencies and other health issues reported by staff.
• Provide timely follow-up regarding the health status of individuals receiving Meaningful Day services following significant changes in condition, including discharge from a hospital or other healthcare facility, to determine the need for additional staff training, nursing interventions, or updates to the Nursing Care Plan.
• Provide written recommendations/guidelines for Self-Medication and share them with the team for individuals with self-medication as part of IP goals.
• Serve as a liaison with home caregivers, families, and Residential Services provider agencies regarding individuals’ health care, for individuals in Day Services as applicable.
• Share teaching of MTTP and Two-Year Clinical Medication Update classes on an equitable basis, which may vary according to MTTP training personnel and schedule.
• Provide teaching, supervision, and support of LPNs, CNAs, Certified Medication Technicians (CMTs), and other Arc staff involved in individuals’ health care.
• Attend staff meetings in all applicable programs when required for staff teaching or other nursing-related issues.
• Communicate with other Delegating RNs, both within The Arc and outside The Arc, regarding health care issues for assigned shared individuals.
• Attend PCP meetings where health issues are a significant/critical part of the PCP process.
• Maintain consistent, thorough, and timely documentation of all nursing assessments, visits, teaching, and interventions.
• Ensure compliance with all DDA and Maryland Board of Nursing policies and directives.
• Maintain timely communication with Residential Program Directors and Coordinators, Meaningful Day Program Directors and Coordinators, Employment Services Program Directors and Administrators, and Community Supports Coordinators regarding participants’ healthcare needs, concerns, and changes in health status, as applicable.
• Provide teaching and counsel for individuals, where applicable, to help them achieve and maintain optimum health.
• Attend a minimum of two DDA Nurses’ Quarterly meetings annually.
• Maintain HRST Clinical Reviewer status.
• Participate in all DDA required training.

Qualifications

• Active RN Licensure in the State of Maryland, preferably with a Bachelor of Science degree in Nursing from an accredited school of nursing.
• Two (2) years of clinical nursing experience, preferably with one (1) year of experience working with individuals with developmental disabilities, and preferably with one (1) year of supervisory experience of unlicensed assistive personnel.
• Thorough knowledge of the Maryland Nurse Practice Act regarding delegation of nursing functions.
• Ability to use basic Microsoft Office applications, including Word, Excel, and Outlook.
• Strong oral and written communication skills and strong interpersonal skills.
• Effective, professional, respectful oral and written communication with all levels of staff, individuals, families, and all other people involved in participants’ care.
