Total Care Services, Inc. is seeking a Coordinator of Community Services (CCS) to support individuals with intellectual and developmental disabilities (IDD/DD) in accessing services, resources, and community-based support.

The role offers a hybrid schedule in Maryland and a full-time position with a base salary of $65,000 per year plus potential bonuses. This is a great opportunity for case managers, social workers, and human services professionals who are passionate about serving IDD/DD

Qualifications

• Degree in Human Services or related field completed.
• 2+ years of case management experience OR 2+ years working with IDD/DD population.
• Must live in Maryland.
• Reliable transportation, valid driver’s license, and active car insurance.

Responsibilities

• Support IDD/DD individuals in accessing services and community resources.
• Serve as liaison between clients, providers and programs.
• Promote independent living and growth through case management.
