Job summary

Mass General Brigham is seeking a Complex Care Community Health Worker to promote and maintain good health within communities. You will educate, support, and outreach to help individuals achieve health goals, including navigation of the healthcare system and basic health screenings.

The role emphasizes collaboration with healthcare professionals, data documentation, and participation in health promotion programs across Hyde Park, Dorchester, West Roxbury, and Wellesley offices with a hybrid work

Qualifications

• High School Diploma or equivalent required.
• Associate's Degree or Bachelor's Degree in related field of study preferred.
• Experience in community outreach, health education, or related field preferred.
• Bilingual skills preferred.

Responsibilities

• Conduct community outreach and engage community members to understand their health needs.
• Provide individual and group health education and coaching to community members.
• Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services.
• Provide basic health screenings and assessments to identify health risks and provide appropriate interventions.
• Assist in the development of health promotion programs and activities to address community health needs.
• Collect data and maintain accurate records of community health outreach activities and interventions.
• Attend meetings and trainings related to community health promotion and education.
