Mass General Brigham is seeking a dedicated community health professional to provide advanced health education, outreach, and care coordination in the community. You will work with healthcare providers to ensure patients receive appropriate services and to improve health outcomes, conducting in-depth assessments of needs and barriers to care.

• Bachelor's Degree in Public Health, Social Work or related field (required).
• 2+ years of experience in community health outreach, health education, or related field.
• CHW Massachusetts License preferred; SDOH and complex patient populations experience preferred.
• Experience helping patients access state/federal programs and benefits preferred.

Responsabilidades

• Provide advanced health education, outreach and care coordination.
• Work with providers to ensure appropriate care and outcomes.
• Assess patient needs, goals and barriers to good health.
• Coach on healthy behaviors and self-management of chronic conditions.
• Collaborate to develop and implement care plans for complex patients.
