HarmonyCares is seeking a Clinical Social Worker in a hybrid role serving Grand Rapids, MI. You will manage complex caseloads, perform psychosocial assessments, and coordinate services to close gaps in care.

The role emphasizes collaboration with care teams and community resources to support home-based care. Qualified candidates hold a Master’s in social work and an LCSW/LISW license, with strong communication, time management, and computer skills.

Qualifications

• Master's degree in social work.
• Licensed as LCSW or LISW in-state.
• Valid driver's license with good driving record.
• Clear written and verbal communication skills.
• Solid computer skills.
• Ability to plan time and balance multiple tasks.

Responsibilities

• Manage a caseload of high-risk socially complex patients.
• Conduct face-to-face and telephonic psychosocial assessments.
• Investigate barriers and design interventions to close care gaps.
• Provide short-term counseling as needed.
• Coordinate services and ensure patient understanding of care plans.
• Collaborate with care team to optimize outcomes and participate in High-Risk Huddles.
• Outreach to community-based services to support aging at home.
• Develop and implement shared goals of care with patient and family.
• Engage community-based agencies to maintain patient independence.
