Job summary

HarmonyCares is seeking a Clinical Social Worker to join our home-based care team serving patients with complex social and behavioral health needs in Florida. You will conduct psychosocial assessments, coordinate services, and provide short-term counseling to support patients and families in navigating care and community resources.

As part of a collaborative care team, you will help design care plans, work with community agencies, and contribute to reducing gaps in care while maintaining

Qualifications

• Master’s degree in social work.
• Licensed as an LCSW or LISW (state dependent).
• Valid driver’s license and good driving record.
• Clear written and verbal communication.
• Above-average computer skills.
• Ability to plan time and multi-task across disciplines.
• Willingness to share expertise and drive improvement.

Responsibilities

• Manage a caseload of high-risk socially complex patients.
• Conduct face-to-face and telephonic psychosocial assessments.
• Investigate barriers and design interventions to close gaps in care.
• Provide short-term counseling as needed.
• Coordinate services and ensure patient understanding of the care plan.
• Collaborate with care team to improve patient experience and outcomes.
• Outreach to community-based services to support aging at home.
• Develop shared goals of care with patient and family.
• Work with community agencies to maintain patient independence.
