Wisconsin Community Services, Inc. is seeking a Certified Peer Support Specialist in Fond du Lac. This hybrid role requires lived experience and Wisconsin certification within 6 months. You will support peers, document interactions in AWARDS, and coordinate with DOC and community resources.

Qualifikationen

• Must have a high-school diploma.
• Must have a valid driver's license, automobile, and insurance sufficient to meet agency requirements.
• Must pass a driving record background check to be eligible to drive in the community during work hours.
• Meet all the employee requirements including references, criminal background check (both WCS and DOC), drug test, and driver's license check.
• Must have completed the State of Wisconsin Peer Support Training curriculum and pass the certification test within the first 6 months of employment.
• One to two years of experience as a peer support is preferred.
• Must have lived experience with substance use or mental health disorders, including justice involvement, and understanding of core Peer Support competencies.
• Knowledge of resources in the community for housing, employment, treatment, food security, parenting, etc.
• General knowledge of supporting individuals in crisis.

Aufgaben

• Maintain a flexible schedule to meet the needs of the individuals served, including being available by cell phone on nights and weekends.
• Provide individual peer support services for people involved in the Fond du Lac- DOC Peer Support Program.
• Link individuals to self-determined supports and services in the Fond du Lac area.
• Support individuals to develop a voice and choice around service participation.
• Maintain accurate documentation of all contacts in AWARDS.
• Assist with facilitation of Motely Crew support group online.
• Turn in program spreadsheets and ensure AWARDS, Billing, and Activity Reports match.
• Complete agency trainings within timelines.
• Attend monthly in-person team meetings.
• Assist with planning community building and recovery activities.
• Attend in-service trainings and conferences.
• Coordinate with outside agencies and DOC for referrals and progress notes.
