Responsibilities

• Review inpatient/outpatient records to verify accuracy of illness severity and medical necessity.
• Ensure documentation reflects patient status, risk of mortality, and care complexity.
• Collaborate with physicians, coders, nurses to improve documentation quality.
• Identify opportunities for improvement in documentation and educate staff.
• Support audits and ensure regulatory compliance.
