Qualifications

• Active, current, valid license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist as applicable by state law.

Responsibilities

• Conducts telephonic and home-visit case management within licensure scope for special programs (e.g., Foster Care).
• Develops comprehensive care plans to address member objectives identified during assessment.
• Coordinates with state agencies and internal/external resources to ensure integrated physical and behavioral health care.
• Addresses social determinants of health and ensures culturally and linguistically appropriate care.
• Educates patients, reconciles medications, and arranges follow-up care.
