Greystone Foster Care is seeking a dedicated Case Manager to join our foster care program in a hybrid role blending remote work with in-office duties. This position supports at-risk youth and families, coordinating services and ensuring regulatory compliance.

The ideal candidate will be flexible for weekends/evenings and possess strong organizational and communication skills. The Case Manager will collaborate with Greystone Foster Care staff and UHS partners to provide a safe, therapeutic

Qualifications

• Bachelor's degree in Social Work or related field; Master's preferred.
• 1–3 years related experience and/or training or equivalent.
• Valid state Driver's License.
• Automobile insurance with verification of medical liability.

Responsibilities

• Complies with state regulatory requirements and Greystone Foster Care plan.
• Ensures that all placement documentation is completed within established timelines.
• Conducts regular file audits and participates in peer audits for audit readiness.
• Assists with completing foster parent reassessments when necessary.
• Schedules child/family team meetings at least quarterly with proper documentation.
• Visits each assigned child per policy or as needed.
• Ensures annual EPSDT and biannual dental appointments are scheduled and communicated to DCS.
• Facilitates outpatient mental health assessments for Level 2 youth and coordinates services.
• Obtains required consents and releases from appointed guardians for medicals.
