Job summary

UCP Oregon is seeking a Personal Agent (Caseload Manager) to support adults with disabilities in Multnomah and Clackamas Counties. You’ll manage a caseload of 36–40 people, coordinate services, advocate for self-determination, and document progress and plans.

The role blends in-person and remote work, with typical hours Monday–Friday, 8:30am–5pm, and occasional overtime. Qualifications include relevant education and driving eligibility; training provided.

Qualifications

• Bachelor’s degree in behavioral science or related field with relevant experience.
• Experience in human services and ability to manage multiple tasks.
• Ability to drive, maintain a valid license and vehicle for site visits.
• Pass reference and criminal history checks, strong commitment to self-determination.
• Demonstrated ability to meet deadlines in a fast-paced environment.
• Must be able to work 40 hours weekly with occasional overtime.

Responsibilities

• Navigate the support services system using person-centered planning to assess needs and develop plans and goals.
• Secure resources and services (employment, housing, crisis, direct support providers).
• Address health and safety needs for caseload individuals.
• Advocate with and on behalf of the people supported; manage plan development and documentation.
• Monitor service quality and ensure independence and choice for clients.
