Job summary

Visions Case Management NM is seeking a Service Provider to strengthen New Mexico communities by supporting clients and collaborating with the team. You will gather information, deliver exceptional services, and empower everyone you interact with.

The role supports monthly consumer contacts, documentation, and program planning. Ideal candidates hold a four-year degree in nursing, psychology, or social work (or equivalents) and can travel across New Mexico.

Qualifications

• Four year degree in Nursing, Psychology, or Social Work with at least 1 year of related experience.
• Alternatively, a four-year degree with 6 years of related experience.
• Willingness to travel throughout the state of New Mexico.
• Strong organizational, time management and communication skills.
• Proficient computer skills including Google Sheets/Drive.

Responsibilities

• Facilitate monthly contact with consumers and submit related documentation promptly.
• Draft and submit annual plans/budgets per program timelines and revise as needed.
• Maintain complete and current client case file.
• Utilize and update internal tracking systems 2 x weekly (Google Sheets or spreadsheets).
• Participate in monthly staff meetings and meet with regional lead weekly or program manager monthly for supervision and training.
• Respond to internal and external communications within 24 hours.
