Job summary

Molina Healthcare in Washington State is seeking a licensed Behavioral Health Transition of Care Coordinator to support Medicaid members transitioning from psychiatric inpatient care. This hybrid role combines work from a home office with travel to local hospitals to meet discharge teams.

You will ensure access to services, coordinate with hospital staff and providers, and focus on reducing readmissions while meeting state licensing requirements and maintaining strong communication with members

Qualifications

• At least 2 years in health care with hospital discharge planning, care management or behavioral health experience.
• Licensed behavioral health clinician with WA license(s) active and unrestricted.
• Valid driver’s license and reliable transportation for travel.
• Knowledge of Care Transitions Intervention (CTI) or similar model.

Responsibilities

• Follow a member through a 30-day program from hospital admission through transitions to other settings to reduce readmissions.
• Collaborate with hospital discharge planners, hospitalists, outpatient providers, facility staff, and families.
• Ensure safe transitions with adequate caregiving, medical oversight and equipment as needed.
• Conduct face-to-face hospital visits and home visits for high‑risk members post-discharge as required.
• Coordinate care and reassess needs using Coleman Care Transition model after discharge.
