Molina Healthcare, Inc. is seeking a Transition of Care Coach (BH Licensed) in Washington state. This hybrid role combines remote work from a home office with local travel to hospitals to ensure effective discharge planning and post-discharge support for Medicaid members.

Applicants must hold an active WA BH license and have experience in discharge planning or BH settings. Travel up to 40-50% may be required, with mileage reimbursement provided.

Qualifications

• Minimum 2 years in health care with discharge planning or BH setting.
• Active Washington BH license (LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD or PsyD).
• Valid driver’s license and reliable transportation.
• Knowledge of Care Transitions Intervention (CTI) or similar model.
• Experience with discharge planning and/or home health.
• Strong communication and time-management skills.
• Ability to work independently in diverse settings.

Responsabilités

• Provide transition of care support for a 30-day discharge program.
• Collaborate with hospital discharge planners, hospitalists, and providers.
• Conduct face-to-face and home visits for high-risk members post-discharge.
• Educate members on transition pillars and care coordination.
• Coordinate interdisciplinary care team meetings and document actions.
