Job summary

Ramey-Estep / Re-group is seeking a Care Navigator to serve as a central contact for behavioral health, primary care, and community resources. This hybrid role blends in-person engagement with telehealth and administrative support to ensure timely, person-centered care.

You will work with clinicians, case managers, and community partners to reduce barriers, coordinate services, and promote recovery, wellness, and long-term stability in line with CCBHC standards.

Qualifications

• High School Diploma or equivalent required; associate/bachelor preferred.
• Experience in behavioral health navigation or case management preferred.
• Strong leadership, organizational, and communication skills; valid driver's license.

Responsibilities

• Welcome and support clients in-person, via phone, or telehealth.
• Conduct initial screenings to determine eligibility and service needs.
• Complete intake forms, consent, and release documents (HIPAA/42 CFR Part 2).
• Educate clients on available services, rights, and community resources.
• Connect clients to behavioral health, primary care, and community supports.
• Track and follow up on referrals to ensure successful connections.
• Collaborate with clinical and case management teams on individualized care plans.
• Maintain ongoing contact through calls, virtual check-ins, and visits.
• Monitor progress, identify unmet needs, and elevate risks to the care team.
• Document interactions and care plans in the EHR; track outcomes with PHQ-9, GAD-7, etc.
