Job summary

Person Centered Services is seeking a Care Coordinator to support individuals with intellectual and developmental disabilities within a health home model. You will coordinate health and behavioral care, community supports, and other services to meet each person’s needs.

The role emphasizes person-centered planning, advocacy, and collaboration with care teams in a hybrid work setting. Training starts with in-person onboarding and ongoing coaching.

Qualifications

• Bachelor's degree with 2 years of relevant experience or RN with 2 years of relevant experience or Master's with 1 year of relevant experience.
• Minimum of a Bachelor's degree is required by OPWDD in NYS.
• Relevant experience includes direct care, coordination, or advocacy such as Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager, etc.

Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their circle of support.
• Manage a tiered caseload and tailor services to individual needs.
• Completes enrollment and eligibility documentation.
• Meets individuals in homes, offices, and public places to conduct assessments and provide services.
• Uphold a respectful, inclusive environment aligned with diversity, equity, and inclusion.
