Job summary

Old Colony Elder Services (OCES) seeks a Home Care Program Care Manager to plan, organize, manage and evaluate HC program operations and supervise care management staff. The role requires oversight of staff and all care-management aspects of the Home Care program, ensuring regulatory compliance.

You will participate in staff development, program analysis and external outreach, maintaining current OCES policies and documentation. A strong leadership and communication background is essential.

Qualifications

• Minimum five years of experience in the human services field, including two years in a supervisory capacity.
• ASAP experience preferred, experience in organization, administration and supervision.
• Excellent written and verbal skills.
• Excellent computer skills; proficient with Microsoft Office Suite or related software.

Responsibilities

• Provide oversight and direction to assigned staff.
• Oversee all care management aspects of the Home Care program.
• Contribute to procedures, standards and guidelines for program operations.
• Ensure program integrity and regulatory compliance.
• Implement quality measures and conduct quality reviews.
• Plan and deliver staff development and training programs.
• Analyze program needs, gaps and service effectiveness.
• Advise with other Home Care management staff; participate in external meetings and outreach.
