Qualifications

• 1 year of care management experience

Responsibilities

• Assess patient needs and develop individualized care plans in accordance with NCQA standards
• Coordinate care across care teams and facilitate patient-centered decision-making
• Educate, coach, and advocate for patients and caregivers regarding treatment options and resources
• Monitor patients across the continuum of care and support transitions to prevent readmissions
• Maintain critical thinking and able to work with minimal supervision in a fast-paced environment
