UCHealth University of Colorado Health is seeking an experienced Registered Nurse for Care Management at the Anschutz campus in Aurora, CO. This onsite role collaborates with physicians and interdisciplinary teams to coordinate care, utilize resources, and promote cost-effective outcomes for diverse patient populations.

Responsibilities include advocating for patients and families, educating on treatment options, and coordinating with care teams to avoid service fragmentation.

Qualifications

• RN licensure in Colorado required.
• CCM certification preferred.
• BLS Healthcare Provider preferred.
• 2 years of nursing experience; acute care and 1 year case management preferred.

Responsibilities

• Demonstrates critical thinking using the nursing process to meet client health care needs.
• Establishes collaborative partnerships to assess health care needs, decisions, and utilization of resources.
• Advocates, educates and coaches clients and families about treatment options and community resources.
• Facilitates communication and coordination between health care team members to minimize fragmentation of services.
• Works in a fast-paced environment with minimal supervision.
