Job summary

UCHealth is seeking a licensed Clinical Social Worker in Aurora, CO to coordinate and manage care for patients with chronic or complex conditions. The role promotes continuity of care, addresses barriers, and supports quality and cost-effective outcomes across the care continuum.

The position is full-time with a hybrid schedule (approximately 50% remote and 50% onsite). It requires NCQA-aligned care plans, strong advocacy for patients, and collaboration with multidisciplinary teams.

Qualifications

• Master's degree in Social Work.
• Licensed Clinical Social Worker (LCSW) license.
• Preferred: Certified Case Manager (CCM).
• Preferred: 2 years of social work experience in an outpatient care setting.
• 1 year of care management experience. Chronic disease management experience.

Responsibilities

• Assesses patient needs and develop individualized care plans in accordance with NCQA standards. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Educates, coaches, and advocates for patients and caregivers regarding treatment options, disease management, and resources.
• Coordinates communication among care teams and facilitate patient-centered decision-making.
• Monitors patients across the continuum of care and support transitions to prevent gaps and readmissions.
• Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Education
