Job summary

UCHealth in Aurora, CO is hiring a hybrid Care Coordinator (LPN/LVN/MA) to provide clinical care coordination, patient outreach, and transitions of care for high-risk patients. The role combines on-site and remote work, with days shift and full-time schedule.

You will collaborate with interdisciplinary teams, support preventive care and education, connect patients with community resources, and help reduce costs while improving outcomes.

Qualifications

• Licensed practical nurse (LPN) or licensed vocational nurse (LVN) or national Medical Assistant (MA) certification required from recognized bodies (RMA/NCMA/CCMA/CMA).
• Graduate of an accredited Practical or Vocational Nursing program or Medical Assistant program.
• Preferred: 1 year of clinical care coordination and healthcare experience.

Responsibilities

• Coordinates patient outreach, appointments, referrals, and transitions of care.
• Supports interdisciplinary care teams with preventive care and care management activities.
• Resolves patient concerns and provides education regarding conditions and care access.
• Connects patients with community resources and office-based care interventions.
