Pay:$22.54 - $33.81 / hour. Pay is dependent on applicant's relevant experience
This is a hybrid position - approximately 50% remote and 50% onsite as work dictates.
Summary:
Provides clinical care coordination, patient outreach, care gap management, and support for high-risk patients to improve outcomes and reduce costs.
Responsibilities:
Coordinates patient outreach, appointments, referrals, and transitions of care.
Supports interdisciplinary care teams with preventive care and care management activities.
Resolves patient concerns and provide education regarding conditions and care access.
Connects patients with community resources and support office-based care interventions.
Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
