Job summary

Mindoula is expanding its Collaborative Care program in Michigan. The Behavioral Health Integration Clinician conducts assessments, develops treatment plans and provides psychotherapy within a team including the medical provider and psychiatric consultant.

You will coordinate care across behavioral and physical health services, track outcomes, document in the EHR, and participate in weekly caseload consultations to optimize patient care.

Qualifications

• Master’s degree in social work, psychology, counseling, rehabilitation, or related field.
• 3–5+ years in behavioral health intervention or care coordination.
• Experience with older adult Medicare population preferred.
• Active independent clinical license in Michigan (LMSW, LPC, LMFT, Psychologist).
• Hybrid work with telehealth; reliable high-speed internet required.
• Proficiency with EHR, scheduling systems and video/phone platforms.

Responsibilities

• Screen and assess patients for mental health and substance use disorders.
• Develop treatment plans and provide psychotherapy.
• Coordinate community and medical/psychiatric care for assigned caseload.
• Utilize clinical technology software for documentation.
• Monitor treatment progress; adjust plans as needed.
• Attend weekly caseload consultations with psychiatric consultant.
• Document progress in EHR and related systems.
