Job summary

Mindoula Health Inc in Michigan is seeking a licensed Behavioral Health Integration Clinician to join our Collaborative Care program. You will assess patients, develop treatment plans, provide psychotherapy, and coordinate mental and physical health care within a primary care setting.

The role requires a master's degree and active Michigan license, 3–5+ years of behavioral health experience, and comfort with telehealth and remote work.

Qualifications

• Master's degree in social work, psychology, counseling, rehabilitation, or other relevant field.
• 3–5+ years of experience in behavioral health intervention clinical work, case management, and/or community care coordination.
• Working with older adult Medicare populations who have chronic medical and behavioral health challenges.
• Active independent clinical license in Michigan (LMSW, LPC, LMFT, Psychologist).
• Current enrollment in the state’s Medicaid & Medicare is preferred; eligibility required.
• Hybrid role with telehealth; stable high-speed internet required.
• Proficiency with EHR, scheduling systems, remote phone systems and video systems.

Responsibilities

• Screen and assess patients for mental health and substance abuse disorders; engage in treatment planning and follow-up.
• Provide brief behavioral interventions using evidence-based techniques.
• Support psychotropic medication management as prescribed by medical providers.
• Coordinate health behavior and social determinants of health interventions.
• Track treatment response and monitor clinical outcomes via EHR.
• Provide patient education about disorders and treatment options.
• Facilitate referrals to evidence-based psychosocial treatments when indicated.
• Participate in weekly caseload consultation with the psychiatric consultant.
• Document progress in EHR and share with providers.
• Meet performance metrics monthly as directed by clinical management.
